The 21st Century Cures Act Final Rule made FHIR R4 a federal mandate rather than a best practice. The CMS Interoperability and Patient Access Final Rule (CMS-9115-F) and its successor, CMS-0057-F, established enforceable requirements for how payers must expose data to members, providers, other payers, and the federal government.
Most organizations are past the question of whether to build these APIs. They are on the question of how to build them so that the endpoints do not just return HTTP 200 during audit season, but actually satisfy the intent of the rules-member-controlled data portability, provider-facing access at the point of care, payer-to-payer continuity of coverage information, and prior authorization that is programmatically inspectable rather than manually opaque.
This is a book for the practitioners who own that work. It is written for the enterprise architect designing the FHIR data layer, the compliance lead answering the auditor's questions, the implementation engineer wiring up the SMART on FHIR authorization flow, and the vendor management team negotiating with the platform providers who now sit inside the regulated boundary.
Chapters walk through each of the three mandated APIs-Patient Access, Provider Access, and Payer-to-Payer-and the CMS-0057-F Prior Authorization API, at the level of detail an implementation team actually needs. The book covers FHIR R4, US Core profiles, SMART on FHIR authorization scopes, the CARIN Blue Button 2.0 and Da Vinci PDex implementation guides, and the connectathon lessons that separate an endpoint that passes a test suite from one that behaves correctly in production.
By the end, you will have a working mental model of the federal interoperability regime, a clear picture of the failure modes your build is most likely to walk into, and the vocabulary to make the case, inside your organization, for the choices that will actually withstand enforcement.
